Why some time disconnect reason is lost-carrier? what is the meaning?

Hi,
You receive such kind of message usually when user session is terminated by device or network failures. Such message can be received
also when user terminates the session by not logging out in a regular way (Turning PC off for example instead of regular log off procedure).
